[A] Budget: Looking for proposal
[B] Scope of Service:
(1) Vendor needs to provide Senior Outreach and Engagement Program Services for older adults who are at-risk due to substance use and behavioural health issues to identify, engage and refer for treatment, as appropriate, so that the older adult can remain integrated in the community in the least restrictive setting possible.
(2) A contract term should be for three years.
(3) All questions must be submitted no later than September 7, 2017.
(4) Last date for letter of intent is September 1, 2017.
(5) A pre-bid conference will be held on September 14, 2017.

Onshore (USA Only)
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office premises.
