[A] Budget: Looking For Proposal
[B] Scope of Service:
(1) Vendor needs to provide pre-employment physical examinations and workers compensation primary care provider services as well as other medical services,
- Optimal medical services to an injured employee in order to return the employee to work as soon as possible after a work related injury or occupational disease, or to provide medical services considered reasonable and medically necessary, in a timely and cost effective manner,
- The provider will give care from initial evaluation to coordination of referrals to network physicians and return to work to the government authority located in Farmington, CT.
(2) All questions must be submitted no later than February 10, 2021.

Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
