(1) Vendor needs to provide miscellaneous medical services to the government authority located in Hartford, CT.
1. Pre-Employment Physical
- Review of occupational and medical history
- Visual acuity and vital signs, including blood pressure, pulse, urinalysis
- Musculoskeletal examination
- Evaluation of the respiratory system
- Gastrointestinal examination
- Examination of head, eyes, ears, nose, and throat
- Skin and lymphatic examination
- Neurological evaluation
- Evaluation of the cardiovascular system
- Drug screening
- Lift Test <50 lbs. (For Specific Administrative Positions Only)
2. PSL Physical and Medical Certification - Based On The Federal Motor Carrier Safety Regulations f 49CFR 391)
- Review of medical history
- Visual acuity (including color) and vital signs, including blood pressure, pulse, urinalysis
- Musculoskeletal examination
- Evaluation of the respiratory system
- Gastrointestinal examination
- Examination of head, eyes, ears, nose, and throat
- Skin and lymphatic examination
- Neurological evaluation
- Evaluation of the cardiovascular system
3. Respiratory physical exam
- Review of respiratory questionnaire
- Visual acuity and vital signs, including blood pressure
- Musculoskeletal examination
- Evaluation of the respiratory system
- Gastrointestinal examination
- Examination of head, eyes, ears, nose, and throat
- Neurological evaluation
- Evaluation of the cardiovascular system
- Pulmonary function test (spirometry)
- Chest x-ray - if deemed necessary by examiner
(2) All questions must be submitted no later than June 26, 2019.
(3) Contract term will be three years.
